System Survey

A System Survey Form must be filled out for all new patients. We recommend taking the survey every six months to check and see your progress or what new symptoms have arisen. The Systems Survey is a questionnaire of symptoms that correlate with the many organs and glands of your body.

While one single symptom does not provide us with much information, a pattern of symptoms can uncover many underlying health imbalances. This is the purpose of the System Survey Form. With this information we can build an individualized program designed to support your primary concerns and health goals.
