We’re super excited to have you as a patient!

If you haven’t already, book your FREE consultation with Dr. Demaray. Click the BOOK NOW button and select “New Patient Consult”

All new patients are required to fill out the following four forms (New Patient Intake Form, The System Survey, The Toxicity Quiz, & The Stress Assess) 48 hours before their appointment. If the forms are not completed by the night before, we will reschedule your appointment to a future date. If you have any issues, we’re more than happy to help!

New Patient Intake Form

Fill out to the best of your ability. This form lets us know who you are, you’re history, and the reason why you’ll be coming in to see us.

The System Survey

A System Survey Form must be filled out for all new patients. We recommend taking the survey every three to six months to check and see your progress or what new symptoms have arisen. The Systems Survey is a questionnaire of symptoms that correlate with the many organs and glands of your body.

While one single symptom does not provide us with much information, a pattern of symptoms can uncover many underlying health imbalances. This is the purpose of the System Survey Form. With this information we can build an individualized program designed to support your primary concerns and health goals

Toxicity Quiz
